TD sleeve glue
3 Rivers recommends this glue for sleeve installation and it works great. It's from the makers of smooth on which is the glue I use in fiberglass bow glue ups. Great stuff.

The answer is yes to both questions. You want to make the wood as close as possible to a slip fit, yet you want a little space for the glue as well. Generally, when you make the wood as close as you possibly can, there is still enough of a gap for the glue.
